ACC Medical Authority Form

For our friends at HRNZ, here you can find the ACC Medical Authority form that was discussed at the “Navigating the complexities of Workplace Injury Management from an HR perspective” webinar held on 15/05/2024.

This form plays such an integral part in proactive claim management. Without it, the employer has very limited ability to discuss the workers claim with ACC, doctors, physios, any medical or allied heath professionals.

You will note that our contact details are inserted in the form as a third party claim manager. You are welcome to leave them there if you would like our support.
